Symptoms of early-stage prostate cancer are mostly unnoticeable. Symptoms generally present only after the disease has spread.

PSA test effective but not perfect,The PSA test and advances in treatment have prostate symptomsled to a 40 percent reduction in prostate cancer deaths since the mid-1990s, and 90 percent of all prostate cancers are now discovered before they spread outside the gland. At this early stage, the survival rate is nearly 100 percent. Without PSA testing, there is no mechanism for early detection of prostate cancer, leaving thousands of men vulnerable and unprepared to fight the disease.

Let us not withhold a test that contributes to early detection of prostate cancer and reduced incidence of related mortality. Let us not recommend against men taking an active role in their own health. Let us move forward by promoting good health, individualized decision- and patient-centered care.

SymptomsChordomas grow slowly. Patients often have symptoms for a long time (more than a year) before they seek medical attention.

Traditional chemotherapy has not been very effective, and radiation alone rarely cures the tumor.

Most chordomas occur at the base of the spine (sacrum), in the tailbone (coccyx) or at the base of the skull (40 percent), but they can occur other places in the spine.

Chordomas are life-threatening. They can cause death by direct growth or by spreading to other organs. They spread to the lungs about 20 to 30 percent of the time.

TreatmentTreatment of chordomas is very difficult. This is mainly because these tumors are near the brain and spinal cord.

Although it is the best treatment for chordomas, in many cases surgery can be difficult to perform because of important structures near the tumor. The surgeon must remove the tumor and a margin of normal tissue around it. This often results in a loss of neurologic function. For example, surgical removal of tumors in the sacrum can result in loss of control of bowel and bladder function (incontinence).

Research on the HorizonAdvances in radiation therapy are allowing higher doses of radiation to be applied with less damage to surrounding normal tissues, such as the brain and spinal cord. Different types of radiation therapy (including proton beam therapy, intraoperative radiation, and brachytherapy) may offer better treatment of these tumors. These therapies may be combined with more limited surgery in the future.

Chordoma chordomas,DescriptionChordomas develop in a structure called the notochord. The notochord forms the early spine in the beginning stages of fetal development. Most of the notochord is replaced by the spine during the first six months of development. Small areas can remain, and chordomas can form in these areas.

Fifty to 75% of patients treated with surgery will be alive after 5 years.

Research on the HorizonChordoma is a rare tumor that usually occurs in the spine and base of the skull. It is a malignant tumor that grows irly slowly. It can spread to other organs, usually the lungs. It represents only about 1 percent of all malignant bone tumors.

Tumors in the spine can cause pain in the area of tumor (neck, back, or tailbone). A tumor pressing on nearby nerves cchordomasan cause symptoms similar to a slipped (herniated) disk. These symptoms include arm or leg pain, weakness, or numbness.

Chordomas themselves do not show up well, but the bone damage may be seen on the x-rays. X-rays of the sacrum are often difficult for doctors to interpret.

Most patients with chordomas are between 40 and 70 years of age. Occasionally this tumor can occur in younger patients, even children. The average age of people affected is about 55 years.

CauseChordomas occur spontaneously. They are not known to be caused by trauma, environmental ctors, or diet. Chordomas are not inherited. Chordomas are not associated with any medical conditions or use of any medications or supplements.

New drug treatments, including use of a drug called imatinib mseylate are being investigated which may help slow the growth of tumors that cannot be removed.

Tumors in the tailbone area can cause a noticeable mass, numbness in the crotch area, and problems with the bowel and bladder.

TestsSeveral imaging tests may be required to identify a chordoma, including x-rays, computed tomography (CT) scans, and magnetic resonance imaging (MRI) scans.

Surgery is the preferred treatment option, where possible. Surgery in combination with radiation is often used to treat chordomas. When tumors cannot be surgically removed, sometimes radiation therapy can stop the growth of the tumor.

There is no currently accepted medical treatment or drug for schwanomatosis, but surgical management is often effective. Pain usually subsides when tumors are removed completely. Genetic testing is available for milies with documented cases of NF1 and NF2 but such testing for schwannomatosis currently does not exist.

For NF2, improved diagnostic technologies, such as MRI, can reveal tumors as small as a few millimeters in diameter, thus allowing early treatment. Surgery to remove tumors completely is one option but may result in hearing loss. Surgery also can correct cataracts and retinal abnormalities.

What is the prognosis?

Several years ago, research teams located the exact position of the NF1 gene on chromosome 17. The product of the NF1 gene is a large and complex protein called neurofibromin, which is primarily active in nervous cells as a regulator of cell division. Intensive efforts have let to the identification of the NF2 gene on chromosome 22. The NF2 gene product is a tumor-suppressor protein called merlin. Ongoing research continues to discover additional genes that appear to play a role in NF-related tumor suppression or growth. Other research is aimed at understanding how the genetic mutations that cause the benign tumors of NF1 also cause nerve cells and nerve networks to form abnormally during fetal development, which later result in the learning disabilities and cognitive deficits of children with the disorder. Additional research is aimed at understanding the natural history of tumors in NF2 and determining possible ctors that may regular their growth patterns. The Interinstitute Medical Genetics Research Program at the NIH Clinical Center conducts NF2 mily history research. Using specimens from some of the milies, scientists have isolated and sequenced the NF2 gene and have described two different patterns of clinical features in NF2 patients. Investigators are continuing to study these patterns to see if they correspond to specific types of gene mutations.

The neurofibromatoses are genetic disorders that cause tumors to grow in the nervous system. The tumors begin in the supporting cells that make up the nerves and the myelin sheath--the thin membrane that envelops and protects the nerves. These disorders cause tumors to grow on nerves and produce other neurofibromatosisabnormalities such as skin changes and bone deformities. Although many affected persons inherit the disorder, between 30 and 50 percent of new cases arise spontaneously through mutation (change) in an individuals genes. Once this change has taken place, the mutant gene can be passed on to succeeding generations. Scientists have classified the disorders as neurofibromatosis type 1 (NF1), neurofibromatosis type 2 (NF2), and schwannomatosis, a type that was once considered to be a variation of NF2. NF1 is the more common type of the neurofibromatoses. In diagnosing NF1, a physician looks for changes in skin appearance, tumors, or bone abnormalities, and/or a parent, sibling, or child with NF1. Symptoms of NF1, which may be evident at birth and nearly always by the time the child is 10 years old, may include light brown spots on the skin (cafe-au-lait spots), two or more growths on the iris of the eye, a tumor on the optic nerve, a larger than normal head circumference, and abnormal development of the spine, a skull bone, or the tibia. NF2 is less common and is characterized by slow-growing tumors on the eighth cranial nerves. The tumors cause pressure damage to neighboring nerves. To determine whether an individual has NF2, a physician looks for eighth nerve tumors, cataracts at an early age or changes in the retina that may affect vision, other nervous system tumors and similar signs and symptoms in a parent, sibling, or child. The distinctive feature of schwannomatosis is the development of multiple schwannomas (tumors made up of certain cells) everywhere in the body except on the vestibular branch of the 8th cranial nerve. The dominant symptom is pain, which develops as a schwannoma enlarges or compresses nerves or adjacent tissue. Some people may develop numbness, tingling, or weakness in the fingers and toes.

In most cases, symptoms of NF1 are mild, and individuals live normal and productive lives. In some cases, however, NF1 can be severely debilitating and may cause cosmetic and psychological issues. The course of NF2 varies greatly among individuals. In some cases of NF2, the damage to nearby vital structures, such as other cranial nerves and the brain stem, can be life-threatening. Most individuals with schwannomatosis have significant pain. In some extreme cases the pain will be severe and disabling.

Delays in the Diagnosis of Lung Cancer

A common theme among people I talk with who have lung cancer is how long it took to be diagnosed. Sometimes the delays I hear about are irly short - the specialist was busy for a week and a half before they could get in. But other times the delays are months in length.

A new study looked at the average delay in the diagnosis of lung cancer among 52 patients. In this study, patients waited on average 21 days to see a doctor, with an additional 22 days needed to complete the investigation. The median time from their first visit to seeing a specialist was 27 days.

The overall numbers were more revealing. On average, from the time people first began having symptoms to the time treatment was begun was 138 days.

I am surprised that you don&8217;t mention CT screening for those at high risk for lung cancer on any of your lung cancer links since that is the ONLY verified way of detecting lung cancer at an early curable stage. You frequently refer to chest x-rays yet the same trial that proved that CT screening can save lives also proved that chest x-rays are virtually useless.

The authors conclusion to this study was that we need to raise awareness about lung cancer symptoms, and expedite the diagnosis process for people who have symptoms of lung cancer. Perhaps if we approached lung cancer (yes, that means more funding which is desperately lacking for lung cancer) as we have breast cancer with regards to symptom awareness and diagnosis, we might see a change in the saddening survival rates for those we love with lung cancer.

An older study revealed similar findings. In that study the median time from first symptoms to the onset of treatment was almost 4 months.

Delays in the Diagnosis of Lung Cancer,

I don&8217;t totally disregard x-rays as an x-ray showed mine. I think a good amount of delay is due to Primary Physicians and their ignorance about Lung Cancer. Even when a patient presents with classic Lung Cancer symptoms, they don&8217;t consider the possibility in an otherwise healthy, NON-smoking patient. This has quite deadly implications and result. Being that Primary&8217;s/GP&8217;s are most often the first line defense..Re-education about the true scope of this disease needs to happen THERE, Until we can deprogram everyone (including healthcare professionals) that smoking is A cause, not THE cause of Lung Cancer..we will continue to see under-diagnosed patients&8230;and mind-blowing death stats. We need CT screening for at risk patients, as well as a total shift in medical profession&8217;s mindset on the subject of Lung Cancer.

The patient&8217;s history&8230;.milial, developmental, environmental exposures, lifestyle/health behaviors&8230;coupled with the pertinent aspects of their presenting symptoms&8230;creates the level of the HCP/PCP&8217;s index-of-suspicion. ~ This presumes professional competence, which sometimes proves toDelays in the Diagnosis of Lung Cancer have been quite a leap of ith. ~ And, yes, skip the X-ray&8230;unnecessary non-definitive rads, unnecessary waste of time and an unnecessary needless bill to pay. Save X-Rays for finding fractures, swallowed metal objects, TSA screenings, etc. ~ Get the lsymptoms of lung cancerow-dose CT w/contrast (STAT in a symptomatic pt.) F/U if warranted with needle biopsy by an interventional Radiologist for cytology, STAT. If malignant, genetic testing of biopsy tissue to allow usage of targeted chemotherapies should also be ordered STAT, if appropriate for cell-typology of the LC found. ~ Discuss the treatment centers with regional/national distinctions in Oncology available with the patient BEFORE passing them on to a local Oncologist, only to have the patient seek them out later for a more comprehensive second opinion. You can often get someone seen quicker @ a major Oncology Center than worked in locally.

As a breast cancer survivor its hard not to wonder if this delay in the diagnosis of lung cancer might play a role in lowersurvival rates. As I look back at my own journey, waiting 138 days - or in the other study almost 4 months - seems totally unacceptable. Almost barbaric.
